What to expect.

Physical Therapy EvaluationAt your first visit, we will thoroughly review your health history, perform an evaluation and exam, and provide you with a diagnosis. We will then perform treatment, and with your help, develop goals and a plan to get you better. Our goal is to get you back to 100% in as few visits as possible. Then, we will help you manage your injury or pain moving forward.

Before your first visit.

Once your first appointment is booked you will receive an email with an intake form and consent forms to sign. Please fill this out before your first visit. If you are unable to do so, or simply forget, don’t worry, you can fill it out when you arrive!

Make a list of questions that you have to discuss with your therapist.

  • List out signs and symptoms that can be of importance to the therapist.
  • Have key past medical history, previous treatments, current medications, and other important information ready.

Payment: Our focus is on you.

We want our time and energy to be focused on getting you better, which is why we are an out-of-network provider with most insurance companies. We offer a self-pay, discounted payment rate that is paid at time of service. If you’d like, we can provide you with a superbill (itemized receipt) to be submitted to your insurance company for reimbursement – or to be applied towards your deductible.

We DO bill Medicare/Medicaid, Auto, and Worker’s Comp insurance plans. 

Our services are covered by HSA/FSA plans. We accept all major credit cards, cash, and check.
